Enrollment Terms
Tuition
Monthly payments, if applicable, are due by the 8th of each month during the term of this agreement. If a monthly payment is not received by the 8th of each month, a late fee of $10.00 will be assessed to Student's account. All tuition, including the tuition down payment and application fee is non-refundable. A $20.00 fee will be assessed to Student's account for any returned checks. If Client wishes to prematurely terminate this enrollment, tuition will be prorated in accordance to length of study and current DSM tuition rates upon completion of the "DSM Withdrawal Form". If DSM prematurely terminates this agreement due to delinquent tuition payments DSM may accelerate the payment of the balance of the tuition.
 
Missed/Canceled/Tardy Sessions
Student can reschedule a session during the week of a regularly scheduled session, a week prior to the regularly scheduled session, or a week after the regularly scheduled session. If Student is unable to attend a session time within this three week period the missed session is permanently cancelled. If Student fails to attend a session without notifying DSM, no rescheduling is permitted. Client is responsible for tuition regardless of whether Student attends any session. DSM does not guarantee a full session time if Student is late to a session.
 
Schedule/Faculty Change
Throughout the duration of this enrollment, requests for schedule changes may be made. If the day, time, and faculty of a requested schedule change are available, DSM will execute the first schedule change free of charge; a fee of $15.00 will be assessed to Client's account for each and any subsequent schedule change. Currently scheduled sessions continue without interruption until the requested change becomes effective. DSM reserves the right to substitute and/or change Student's Faculty at any time.
 
Instruments and Supplies
With the exception of pianos and percussion instruments, Student must supply his or her own instruments; amplifiers will be available for electronic instruments. Student is responsible for educational supplies and teaching materials assigned by the Faculty, such as books, reeds, and sheet music.
 
Miscellaneous
The term of this agreement is the period of time set forth in this agreement, unless terminated earlier at DSM's sole discretion without prior notice upon one of the following events: (a) disruptive conduct by Client/Student at DSM's place of business; or (b) the failure of Client to pay tuition as documented in this agreement.
